Ongoing
Ongoing Volunteer Requests - Registered Member Agencies, use this form to request volunteers for positions that are regular in nature and on-going. (Not One-Time Projects)

One-Time Projects
One-Time Project for Volunteers [doc] mail-in Form. Registered agencies can use this form to request volunteers for special projects that require volunteers for one shift or several shifts for a couple of months. (MINIMUM 2 WEEK NOTICE REQUIRED!!!!) This form can be downloaded to your computer and filled out. Completed forms can be emailed, mailed or faxed to Mesa County RSVP, Inc.
Timesheet
Timesheet [doc] Track your RSVP volunteers' time. This will save the volunteers from having to do individual timesheets. Just remember to have the volunteer sign each day they are at your site and have a representative from your agency sign the form.
