Mail-in Member Agency Application

To be able to request volunteers from Mesa County RSVP, your agency needs to have completed the registration process. Filling out and mailing in this application the first step in this process.

Mail-in Agency On-going Volunteer Request Form

Registered Member Agencies, use this form to request volunteers for positions that are regular in nature and on-going. (Not One-Time Projects)

Mail-in "One-Time Project" Volunteer Request Form

Registered agencies can use this form to request volunteers for special projects that require volunteers for one shift or several shifts for a couple of months. (MINIMUM 2 WEEK NOTICE REQUIRED!!!!) This form can be downloaded to your computer and filled out. Completed forms can be emailed, mailed or faxed to Mesa County RSVP, Inc.

Mail-in Agency Group Timesheet

You can use this timesheet if you are one of our registered agencies to track the volunteer time of your RSVP volunteers. This will save the volunteers from having to do individual timesheets. Just remember to have the volunteer sign each day they are at your site and have a representative from your agency sign the form.
