Mail-in Volunteer Registration Form

Use this form to register to become a volunteer with Mesa County RSVP.

RSVP Photo Release

Mail-in this form. All registered RSVP volunteers need to indicate whether they do or do not give permission to RSVP to use photos of them to promote RSVP.

Individual Volunteer Timesheet Form

Mail-in this form. It tracks your volunteer time to RSVP each month.

Application for Mileage Reimbursement

In order to be eligible to receive Mileage Reimbursement for the miles driven to and from your volunteer site, you need to complete this application form and mail it to Mesa County RSVP, Inc. before you can claim any reimbursement. Reimbursement is NOT retroactive and begins from the date your application is received.

Volunteer Mileage Reimbursement Form

Main-in this form. You must submit it monthly in order to receive mileage reimbursement as a registered member of Mesa County RSVP.
